It does not cover clinical information shared with your therapist once you become a client; that information is governed separately by our clinical privacy practices and the Notice of Privacy Practices provided at the start of care.

What We Collect

When you submit a consultation form, we collect the information you choose to provide: your first name, email address, optional phone number, optional insurance information, and any brief note you include. We also record the page where your visit began and general referral or campaign information so we can understand how people find us. Our website host, Netlify, may process form submissions and basic technical information such as browser type and IP address to operate the site and screen for spam.

How We Use It

We use the information you submit to respond to your inquiry, discuss fit, match you with a therapist, and check insurance details when requested. General source and landing-page information helps us understand which outreach efforts lead to consultation requests. We do not send the contents of your form to advertisers, and we do not sell your information.

Who We Share It With

Information is processed only by NYSOM and the service providers needed to operate the website and respond to inquiries, currently including Netlify and our email provider. We do not share form contents with advertisers or data brokers.

Cookies & Tracking

This website uses Google Ads measurement to understand whether an advertisement leads to a completed contact form. Google may set or read cookies and receive basic device, browser, referral, and conversion information for that purpose. We do not send the contents of your contact form to Google, and we do not sell your information. How Long We Keep It

Information submitted through this website is kept only as long as reasonably needed to respond to your inquiry and maintain appropriate business records. Once you become a client, clinical records are handled separately under NYSOM's clinical privacy and recordkeeping practices.

Your Choices

You can ask us to delete information submitted through a website form, subject to any legal or professional recordkeeping obligations, by emailing hello@nystateofmindpsychotherapy.com.

Website forms are intended only to begin a conversation. Please do not use them for detailed health or clinical information, emergencies, or urgent concerns. If you are experiencing an emergency, call 911 or go to the nearest emergency room.

Children

This website isn't directed at children, and we don't knowingly collect information from anyone under 18 through our contact form.
