What decades of outcome research actually say about what makes therapy work.

Ask most people what makes therapy effective, and they will point to technique: the right modality for the right problem, deployed correctly. Technique matters, but it is not what the outcome research points to first.

A 2018 meta-analysis of 295 studies by Flückiger, Del Re, Wampold, and Horvath, published in the journal Psychotherapy, found that the strength of the client-therapist alliance reliably predicts outcome across treatment types and presenting problems. That same year, a landmark review by Norcross and Lambert, synthesizing decades of research under the title Psychotherapy Relationships That Work, concluded that the relationship itself contributes to outcome at a level comparable to the specific technique being used.

None of this means modality is irrelevant. A trauma-focused approach and a couples-focused approach are built for different work, and matching the right framework to the right problem still matters. But it reframes the first question a practice should be asking, from which modality is most impressive to whether this therapist and this client can build a relationship strong enough to do the work at all. That is the question we start with.
